St. Anthony Hospitals Senior Benefit Program is available to help people with Medicare make decisions about Medicare Prescription Drug Coverage (Part D). Everyone with Medicare must make a conscious decision regarding drug coverage. This benefit applies to everyone with Medicare, regardless of income and whether or not you currently take prescription medications.
If you would like help to understand the coverage or if you are having trouble deciding which plan best meets your needs, we are here to help.
To receive a personalized report on which Medicare prescription drug plan option is best for you, please complete the Medicare Part D Intake Form - please print legibly. You may want to get a listing of your current prescription drugs from your pharmacy and attach it to the form. Then mail the form to us at:
