ReConnect is an innovative program at Centura Health that offers assistance to hospital patients and community members to help them achieve their health goals. Volunteer coaches provide a caring personal touch and practical assistance to people who may be transitioning from hospital to home, living with a chronic health condition, or simply seeking information about navigating the health care system.

People Matter
Health care delivery systems are increasingly challenged to meet the needs of the communities they serve. Pressure to be more efficient and control costs sometimes leaves less time for that personal touch that everyone wants and which we know improves health outcomes. The ReConnect program helps fill this gap by giving extra time to listen and connect people with available resources they may need whether inside the hospital or out in the community.
ReConnect coaches are valued members of the health care team who assist patients in achieving their health-related goals. In addition to improving the quality of the hospital experience, coaches provide skills and tools that empower patients and families to better manage the health challenges they face outside the hospital.
Challenging Opportunities
Coaches are passionate, unpaid civic leaders who come from all walks of life. They share a passion for learning and applying their knowledge and life experience to help patients and families with effective use of the health care system.

Volunteers receive 36 hours of training, including classroom instruction and shadowing to observe coaching in action.
